Definitely it shouldn't. A template like

<xsl:template match="chapter_title">
  <fo:block font-size="14" font-weight="bold">
    <xsl:value-of select="."/>
    <xsl:apply-templates select="sub"/>
  </fo:block>
</xsl:template>

will give you the effect you described, but not the one below.
